  • Medical Certificate
  • Certificate of Detention
  • Three (3) copies of 2×2 colored ID photo taken within one month from remittance, with full name tag in bold letters, plus left and right profile of the same specifications and number of copies
  • Mittimus order, if convicted prisoner
