Monday  – Friday; 8:00 AM – 5:00 PM

Who may avail of the Service:

Indigent patient confined in a government hospital
Distressed, displaced individual

Requirement:

• • • Referral letter
    • Medical certificate and doctor’s prescription (in case of medical assistance)
    • Blood request duly signed by th attending physician (in case for blood processing)
    • Death certificate for transfer of cadaver (in case of request for transportation)
